The ActaStim Sync Application connects via Bluetooth to your prescribed medical device (the “Device”) to obtain data relating to the usage of the device and your activity level (“Device Inputs”). The application will also invite you to track pain levels, activities, comments and other such information as may be relevant to your treatment (collectively, “User Inputs”). In order to provide continued access to the Application, all such Device and User Inputs will be backed up and stored in a secure HIPAA compliant database environment.
Identifiable (confidential patient) Inputs that are linked to your personal details will not be accessible to others except for trained Theragen representatives who are responsible for ensuring the proper use and function of the Device and Application or enhancing your experience with the Application.
We do not share the Identifiable Inputs or any data related to your operation of the Application (collectively, the “Data”) with any third party except for 1) your healthcare provider and their team and 2) those with whom Theragen is directly contracted, who are confirmed to be HIPAA compliant and who require access to Data only for the purposes of maintaining the Application. Your data will never be sold.
We may de-identify the Data (remove your personal details) for the purpose of statistical analysis that may be shared with researchers, prescribers, healthcare professionals, reimbursement community representatives and other interested parties (Collectively, “Interested Parties”). By accessing the Application, you agree that you are giving permission for de-identified Data to be shared with Interested Parties. I understand that Interested Parties will not be provided with my name or any other information that would identify me personally.
